In what order do people write the address for a letter to Great Britain, America, Australia?

In the centre of the front side of an envelope we write:

the name of the addressee;

the number of the house, the name of the street and the number of the apartment (flat);

the name of the town (vil­lage);

the name of the country (it is unnecessary if the town is well-known).
