Atherosclerosis and hypertonic disease
Atherosclerosis risk factors
Age (frequency increases with age)
Sex (more frequent in men)
Family predisposition
Hyperlipidemia (hypercholesterolemia) – LDLP/HDLP > 4
Hypertension
Smoking
Diabetes mellitus
Also important:
Stress
Hypodynamia
Obesity
Hyperurikemia
Oral contraceptives (especially associated with smoking)
Arteriosclerosis types
Atherosclerosis (metabolic arteriosclerosis)
Hyalinosis (e.g. hypertonic disease)
Inflammatory arteriosclerosis (syphilitic, tuberculous)
Allergic arteriosclerosis (e.g. nodular periarteriitis)
Toxic arteriosclerosis (e.g. adrenaline)
Primary calcinosis of the arterial media (mediacalcinosis of Menkeberh)
Senile arteriosclerosis
Modified lipoproteins aterogenous action mechanisms
Captured primarily by scavenger-receptors of macrophages
Stimulate chemotaxis of the circulating monocytes
Activate monocytes adhesion
Limit the mobility of the macrophages and thus contribute to the accumulation of macrophages in the site of damage
Growth factors and immunokines synthesis activation
Cytotoxic to the myocytes endothelium
Macroscopic stages of atherosclerosis
Fatty spots and stripes
Fibrous plaques
Complicated damage
Ulcerations
Hemorrhage
Thrombosis
Calcinosis
Morphogenic stages of atherosclerosis
Prelipid
Lipoidosis
Liposclerosis
Atheromatosis
Ulceration
Atherocalcinosis
The cause of ulceration is usually the ischemisation of the plaque covering due to the increase of the plaque size.
Vascular changes in atherosclerosis:
Slow narrowing of the nourishing artery opening by the atherosclerotic plaque causes dystrophy and atrophy of the parenchymal tissue
Fast thromboembolisation causes infarction and gangrene
Clinico-morphological forms of atherosclerosis:
Aorta atherosclerosis
Aneurism
Aorta
Thrombosis
Coronary arteries atherosclerosis
Ischemic Heart Disease
Brain arteries atherosclerosis
Acute
Ischemic infarction
Hemorrhagic infarction
Brain cyst
Dystrophy and atrophy
Renal atherosclerosis
Thrombosis, infarction
Cuneiform atrophy
Large nephrosclerosis
Intestinal arteries atherosclerosis
Ischemic colitis
Intestine gangrene
Lower limbs arteries atherosclerosis
Thrombosis, gangrene
Lipoidosis and previous stages are reversible. Liposclerosis and further stages are irreversible. Smooth-muscle tissue covering is atrophic. Atherocalcinosis (dystrophic).
Hypertonic disease is the chronic disease.
Main hypertonic disease risk factors:
Hereditary predisposition
Chronic psychoemotional stress
Excessive salt intake
Vascular changes in hypertonic disease
